Fort Medical Equipment

Was provided a walker from hospital. When contacted by mark who said he was manager came to my house and requested I sign a form that allowed him to request my medical history. uncomfortable with this I did not sign that portion of the form. He became bilgerant and began yelling. My wife was frightened to the point of calling the police.. we filed a complaint with the main company. They were very kind and apologetic.
